Practice Area

In order to register with us, you must live within our catchment area.

If you would like to check whether you are resident within our Practice area and so eligible to register with us a Practice then please check the map below, or fill in your postcode.

How to register with us

You can register with our practice online. If you have any queries, or are unable to register online, please contact reception.

Please be aware that any replies from the surgery may appear in your Junk Inbox.

Alternatively, you can fill in a physical registration form:
  • download and fill out a PRF1 form (PDF) and return it to us
  • collect a PRF1 form from within any of our branches, fill it out and return it to us

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Overseas visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.

Temporary Registration

If you’re currently staying with a friend or family member within our GP surgery area for more than 24 hours but less than 3 months, you can ask to register as a temporary resident. You can find more information on registering as a temporary resident on the NHS website.
