MEDLINK

From the 1st of April 2025, Holt Medical Practice will be using a new system called MEDLINK for reviewing long-term conditions (LTCs) online.

Please click this link to find out more.

Appointments

BEFORE you contact your GP surgery:

Can a pharmacist help me?

Your local pharmacist may be able to offer treatment and some prescription medication for some conditions, without you needing to see a GP.
Conditions they can treat include:

If you go to a pharmacy with one of these conditions, the pharmacist will offer you advice, treatment or refer you to another healthcare professional if needed.

Request a non-urgent appointment via our online form

Please click the button above to use our online form. Your request for an appointment will be triaged by the Surgery and someone will get back to you as soon as possible.

Please ensure you check your emails regularly for our response.

Urgent appointments

To request an urgent appointment (i.e. for today or tomorrow, Mon – Fri) you need to phone the Reception Team on 01263 712461.
You will be asked to share details about your symptoms and concerns with the Receptionist.

For urgent appointments you may be offered a telephone call-back from the Duty Doctor or an appointment with a Paramedic, Physician Associate or Nurse Practitioner.

We do not offer a walk-in service.

Non-urgent or routine appointments

Unplanned routine appointments are available up to 2 weeks in advance.

You can request routine appointments by completing our online form or calling the Reception Team on 01263 712461.

You will be asked to share details about your symptoms and concerns with the Receptionist.

MEDLINK

MEDLINK is a system for reviewing long term health conditions.
It sends invites to relevant patients around their birthday month, usually either through email or text.
If they are not able to receive either of those, they’ll receive an automated call to contact patient admin.

Included in the invite there is a link to an online review which the patient fills out in their own time and if required will be asked to book any physical checks through the NHS APP.
Once all of the patient’s initial investigations are completed and have been reviewed by a health care professional, if any follow up is required the patient may be asked to book a further appointment through the NHS APP.

Please click here for more information.

Requesting a home visit

If you are clinically housebound, you can request a home visit. Clinically housebound means someone is unable to leave their home without significant assistance or effort due to a physical or psychological condition.

To do this, you need to call the Reception Team on 01263 712461, BEFORE 10am.

The Receptionist will ask you for details about why you require a home visit as well as any specific home access instructions (e.g. hidden driveway or key safe code).

Cancelling or changing an appointment

To cancel your appointment:

  • visit the NHS App
  • follow the link included in your appointment reminder text
  • call Reception on 01263 712461

When you arrive at the surgery

Information about our facilities and parking can be found here.

Check in for your appointment using our Check-in Screen. This helps reduce queues and waiting times.

If you have waited over 20 minutes after your appointment time, please let Reception know who will investigate the reasons for the delay.

If you arrive late for your appointment, it will hopefully still be possible to see you, but you may have to wait until the end of the clinician’s list.

If you need help with your appointment

Please tell us when booking your appointment:

  • if you would prefer a telephone call or face to face appointment (please note some appointments may need to be face to face)
  • if you need an interpreter
  • if you have any other access or communication needs
  • if completing an online form, tell us if there is any specific member of our team that you would like the information shared with
