From the 1st of April 2025, Holt Medical Practice will be using a new system called MEDLINK for reviewing long-term conditions (LTCs) online.
When you require a review, which we aim to be around your birthday month,  you will receive an invite containing a link to complete by email or SMS. If you do not have access to either of these, you will receive an automated voice call asking you to contact Patient Administration at the Practice to book in for your review.

Below are example screen shots of the initial email or text you will receive. When you receive the email or text please click on the link and complete your online review. This can be completed on a smartphone, tablet or computer, you can copy or type the link into a desktop computer browser to use a larger screen.

Once you have clicked the link you will be given more information about the online review you may need to compete (screenshot below).

Your responses are saved automatically as you go, so if you can’t complete it in one sitting, you can return at any stage by re-clicking on the link again in your email or text and continue at a later date. Feel free to ask a family member or friend to help you answer the questions.

After completing your review online, you may receive instructions if a physical check appointment (for example, a blood test or blood pressure) is needed.

Using NHS App for MEDLINK appointments

If a physical check is needed, please follow the instructions. These will direct you to the NHS APP to book the relevant appointment as soon as possible, but at a date and time that is convenient to you.

When we have all the information needed to assess your conditions, a clinician will then review your online responses and physical check results and decide if further interaction or discussion with you is required.
We will email or text you to let you know next steps, if we would like to speak with you on the phone or we would like to book an appointment to see you face to face we will send you the information you need to book the relevant appointment on the NHS app. Or we may decide that we are happy with your control and will be in touch in another year.

If you require help with the NHS APP – we have monthly drop-in sessions at the Surgery where you are able to come along with your device and we will assist you with your questions or help to navigate the APP and feel more comfortable using the system.  The next drop-in session is on Tuesday 29th April at Holt between 10am and 12noon.
