Unfortunately, we are not able to register patients living out of our catchment area. This is to ensure we have enough capacity and resources to cover all our patients within our very large area.

How to register with us
It is helpful to have your NHS number ready when you register. You can use the NHS website to find your NHS number.
The easiest way to register with our practice is to click this link and complete the process online.

Alternatively, you can fill in a physical registration form:
- download and fill out a PRF1 form and return it to us
- collect a PRF1 form (from either Holt or Melton Constable Surgery), fill it out and return it to us
Completed PRF1 forms will need to be returned to Reception at Melton or Holt Surgery.
Once you are registered you will receive notification from us via a letter which will include information such as your named GP and how to obtain any repeat medication. If you are having any problems or have any concerns about registering with us you can contact us via our online form (where you can ask your questions) or call Reception on 01263 712461.
Temporary Registration
If you’re currently staying with a friend or family member within our catchment area for more than 24 hours but less than 3 months, and you need acute medical care you may be able to register as a temporary resident. .
Patients are expected to contact their own GP Surgery in the first instance to ask for remote support (over the telephone or online). This can be a much safer method of care as your usual surgery’s healthcare professionals will have full access to your medical records.
This may not be appropriate for all conditions or more urgent situations, in which case anyone looking to register as a temporary resident should visit the Holt Reception in person during opening hours, or call the Reception team on 01263 712 461.

Registering with a GP as a temporary resident
Overseas visitors
Those who visit the UK from overseas must pay for their care when they are in England.
In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.
