Registering with the surgery
To register with the surgery:
- download and fill out a GMS1 form (PDF) and return it to us
- collect a GMS1 form from within any of our branches, fill it out and return it to us
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
